For adults with Medicaid: This program is in contract with Community Health Connections Health Home to provide Health Home Care Management services to adults with Medicaid wo have a severe and Persistent Mental Illness. The Care Coordinators work to access service based upon the individual’s needs for: mental health, physical health, vocational, financial, housing, and social services. This program strives to coordinate the total care of each person resulting in a decreased need to access emergency department and/or inpatient care. A population served by this program are those designated for Assisted Outpatient Treatment (AOT) utilizes courts to mandate treatment for adults who are determined to be non-compliant with medications and treatment posing imminent risk to self or others.
For adults without Medicaid: same eligibility and services as above.
For children and youth with Medicaid: This program is in contract with the Children’s Health Home of Upstate NY (CHHUNY) serving children and youth residing in Rensselaer County who experience Severe Emotional Disturbance. The Care Coordinators work to access care based upon the needs of the child, youth and family. Services to be accessed and coordinated are: mental health, physical health, vocational, financial, housing, and social services. Contained within the program is the High-Fidelity Wrap-around program for youth experiencing a high acuity of emotional disturbance. The goal of the program is to coordinate care in order to decrease the need for emergency dept visits, inpatient hospitalization and out of home placement.
For children and youth without Medicaid: same eligibility and services as above.
