A team of Social Work Aides and Care Coordinators delivering care to persons age 60+ years old residing in the communities of the county and experiencing mental illness or emotional distress. Recipients may reside in Private Proprietary Homes for Adults, own home, or with friends/relatives. This program does not serve residents of nursing homes, or assisted living.
