Community Services

In addition to the services offered by The Rensselaer County Department of Health, residents living in Rensselaer County have a wide range of services available to them. Please familiarize yourself with what is available to you. A brief description is provided for each area with links to more detailed information.

Social Services:
The Department of Social Services offers a wide range of services. The Raddock Building at 1801 Sixth Avenue, Troy, houses service units that provide financial assistance such as Public Assistance, Food Stamps, Medicaid and the Home Energy Assistance Program(HEAP). The adult services unit, formerly located at the Administrative Building, is now also stationed in the Raddock Building. Also available at this location is assistance in pursuing Child Support and Home Health Aide Services for elderly and disabled Medicaid recipients.

The Administration Building at 133 Bloomingrove Drive, Troy, provides an array of casework services such as Child Protective, Foster Care, Adoption and Day Care.

The Department also provides services to young parents through the Healthy Kids Program at Samaritan and the Teenage Opportunity Program at the Commission on Economic Opportunity (CEO).

Family Health Plus:
Family Health Plus is a public health insurance program for adults between the ages of 19 and 64 who do not have health insurance — either on their own or through their employers — but have incomes too high to qualify for Medicaid. Family Health Plus is available to single adults, couples without children, and parents with limited income who are residents of New York State and are United States citizens or fall under one of many immigration categories.

Family Health Plus provides comprehensive coverage, including prevention, primary care, hospitalization, prescriptions and other services. There are no costs to participate in Family Health Plus. Health care is provided through participating managed care plans in your area.

Family Health Plus is available to single adults, couples without children, and parents with limited income, who are between the ages of 19 and 64, are residents of New York State and United States citizens or fall under one of many immigration categories. If you have health insurance — either on your own or through your employer — you are not eligible to enroll. There are no limits on the amount of assets or resources (such as a bank account) that you can have and still be eligible to enroll in Family Health Plus. How much income you and your family can have and still be eligible for Family Health Plus depends upon how many people are in your family.

Child Health Plus:
New York State has a health insurance plan for kids, called Child Health Plus. Depending on your family’s income, your child may be eligible to join either Child Health Plus A (formerly Children’s Medicaid) or Child Health Plus B. Both Child Heath Plus A and B are available through dozens of providers throughout the state. Enrolling in Child Health Plus is easy.

To be eligible for either Child Health Plus A or B, children must be under the age of 19 and be residents of New York State. Whether a child qualifies for Child Health Plus A or Child Health Plus B depends on gross family income. Children who are not eligible for Child Health Plus A can enroll in Child Health Plus B if they don’t already have health insurance and are not eligible for coverage under the public employees’ state health benefits plan. Check the following income charts to see whether your child will most likely qualify for Child Health Plus A or Child Health Plus B.

Elderly Pharmaceutical Insurance Coverage (EPIC) Program:
What is EPIC?
EPIC is a New York State sponsored prescription plan for senior citizens who need help paying for their prescriptions. Over 310,000 seniors already belong and are saving over half the cost of their medicines.

Who can join?
New York State residents can join EPIC if they are 65 or older, and have an annual income of $35,000 or less if single, or $50,000 or less if married.
Seniors who receive full Medicaid benefits or have other prescription coverage that is better than EPIC are not eligible for EPIC benefits.

The Capital District Child Care Coordinating Council:
The Council is a non-profit organization serving parents, employers and child care providers in Albany, Rensselaer, Saratoga and Schenectady counties in New York State. It is a resource and referral agency, providing resources to the community by improving the availability and quality of child care, and a referral service counseling parents seeking child care.

Healthy New York:
The State of New York is committed to quality health care for all of its residents. In 2000, Governor George E. Pataki proposed, and the Legislature enacted, important legislation that has increased the availability of comprehensive health insurance coverage for New York’s uninsured workers and their families. The Health Care Reform Act of 2000 introduced a program entitled "Healthy NY" which promotes access to quality health care by providing comprehensive health insurance to those citizens who need it most.

The Healthy NY program is designed to assist small business owners in providing their employees and their employees’ families with the health insurance they need and deserve. In addition, uninsured workers whose employers do not provide health insurance may also purchase comprehensive coverage directly through the Healthy NY program.

The Prenatal Care Assistance Program:
The Prenatal Care Assistance Program (PCAP) can help you get the care you need for you and your baby. PCAP offers complete pregnancy care and other health care services to women and teens who live in New York State. The Medicaid Obstetrical and Maternal Services (MOMS) Program provides complete pregnancy services in areas of the state where PCAP health centers are not located. And there’s no cost to eligible women who participate in MOMS or PCAP!

Who can get PCAP/MOMS Services?
Women and teens who live in New York State who are pregnant and meet certain income guidelines can receive complete medical care during pregnancy, delivery and for at least two months after delivery. You may be eligible even if you have health insurance.

Medical Examiner:
The office of the Medical Examiner has a chief Medical Examiner who is certified as a forensic pathologist performing all necessary autopsies.

Wellness:
Presentations and/or general information is available by request on a variety of health topics. Programs may be provided to individuals, community groups, schools, businesses and health providers.

Topics include but are not limited to: HIV/AIDS, STD’s, Rabies, Diabetes, West Nile Virus, Healthy Heart, and Disease Education.
