The Supportive Case Management program is a component of the Unified Services Program for Adults, which serves the needs of Rensselaer County mentally ill residents who live in Adult Homes. Most will have significant histories of mental illness. This program is Medicaid billable and operates under an agreement with the New York State Office of Mental Health. A person would need to be referred by a case manager or self refer this program.
